How GKV Contributions Work

In Germany, statutory health insurance contributions are shared between employer and employee. Your total cost consists of four components:

  • Health insurance — employee share (half of the total health rate)
  • Health insurance — employer share (half of the total health rate)
  • Long-term care insurance — employee share (half of the base rate + childless surcharge if applicable)
  • Long-term care insurance — employer share (half of the base rate)

Self-employed individuals pay the full contribution themselves — there is no employer subsidy. Contributions are always calculated on your gross income, capped at the assessment ceiling of €5,812.50/month (2026).
