What is Heilfürsorge?
Heilfürsorge is a form of full state healthcare provided to certain civil servants — primarily police officers, military personnel, and fire service members in some federal states. Unlike Beihilfe, Heilfürsorge covers 100% of medical costs during active duty.
Critical warning: Heilfürsorge rules are highly state-specific. Not all police, military, or fire service groups receive Heilfürsorge in all states. The transition conditions, timing, and eligibility criteria vary significantly. Do not assume that rules from one federal state apply elsewhere.
The transition problem
When Heilfürsorge ends — typically upon promotion, retirement, or a change in status — you transition to Beihilfe. At that point, you need Beihilfe-compatible private insurance for the first time.
The challenge: if you wait until the transition to apply for insurance, you may face:
- Higher premiums — because your entry age is now higher than when you first entered service
- Health-based surcharges or exclusions — because conditions that developed during service are now pre-existing
- Limited tariff options — because some tariffs may not be available at older entry ages
The solution: Anwartschaftsversicherung
An Anwartschaftsversicherung (option insurance) preserves your right to transition into a Beihilfe-compatible tariff under favorable conditions — even years before you actually need it.
Two types exist:
Small Anwartschaft (kleine Anwartschaft)
- Preserves your health status at the point of entry — conditions that develop later cannot lead to surcharges or exclusions
- Very low monthly cost — typically a few euros
- Does not preserve your entry age for premium calculation
- Suitable as a minimum protection strategy
Large Anwartschaft (große Anwartschaft)
- Preserves both your health status and your entry age — premiums are calculated as if you entered the tariff at the original start date
- Higher monthly cost — but substantially lower than delaying entry by 10–20 years
- Begins building aging reserves during the Anwartschaft period
- Recommended for civil servants who are certain they will later need Beihilfe-compatible cover
When does Anwartschaft make sense?
An Anwartschaft is most valuable when:
- You currently receive Heilfürsorge and will later transition to Beihilfe
- You are young and healthy — locking in these conditions early provides maximum long-term value
- Your federal state's transition rules are clear and predictable
- You want to avoid the risk of health-based complications at the point of transition
